You need this information to file an application for year 2003 rent and property tax rebates.
Proof of income for 2003
(Social Security, SIS, Pensions,
Rental Income, Bank Interest
Receipt for Paid 2003 Property Tax
Rent Receipts for 2003 with the landlord or rental agent's name, address, telephone number and social security number. (If this information is not on the receipt you must have the rental agent complete a part of the rebate application or have the information available for the prepare to write on the form.)
FIRST TIME APPLICANTS FOR REBATE ONLY:

If disabled: Proof of Disability

the hours of 9:00 a.m. and
12:30 noon & 1:00 p.m. - 3:30 p.m.
